Medical errors can save lives as they elucidate where our procedures and systems fail to protect our patients. This lecture covers systems-based approach to error and root cause analysis – determining why an error really happened, with many of the contributing factors far beyond the point of care to the patient. Case examples are discussed to demonstrate key principles.
